Did you or someone you know suffer specific, tangible harms or damages as a result of Alameda County's COVID state of emergency/COVID policies? Examples include:

• Business that had to close due to shut down orders
• Landlord whose tenants didn’t pay rent and couldn’t be evicted due to eviction moratorium
• Person with serious adverse reaction to COVID shot
• Alameda County worker who lost their job due to vax/testing/mask requirements
• Student unable to enroll in Peralta community classes due to vax/testing/mask requirements 

If so, please fill out the form so that we can gather these important facts. 

Note: Submitting a declaration does NOT enter you into this lawsuit/Writ of Mandamus. Your declaration will make your grievances known to the court and be included in documents submitted to the court, therefore County counsel and the County will see it. However, the declarations are only supplementary evidence used to show real harms and damages were inflicted on the people of Alameda County. We don’t expect info from declarations will get used for anything beyond evidence for the court.
